The Formulary, or Preferred Drug List (PDL).

The formulary lists common preferred medications that are generally covered under your prescription drug benefit. It is not a guarantee of coverage, and it is not an all-encompassing list. The formulary development process combines clinical and financial decision-making to create a preferred drug list that delivers clinical integrity and competitive pricing within the pharmacy benefit. Covered medications will fall into a drug tier for copayment based on your plan’s formulary.

The Pharmacy & Therapeutics committee reviews new medication to determine formulary placement and drug tier. Some medications are excluded from coverage due to prescribing guidelines and financial factors. The formulary is just one of a full suite of Pharmacy Benefit Management services available. Formularies are checked multiple times yearly to ensure that they stay up to date with current market standards.

Medications are grouped into tiers based on common use, brand status, and overall value profile. Select a tier below to learn more.

First-tier medications are generally the generic formulations and the most common medications available.

Second-tier medications are the more common brand name medications. They provide better clinical and economic value than higher-tier counterparts.

Third-tier products are specialty medications that treat chronic and rare conditions, as well as non-preferred brand names with a lower value profile.
